As it is customary, in this issue of our journal “Clujul Medical” we include – following a rigorous evaluation and careful selection – a variety of valuable and interesting reviews and original scientific papers. All biomedical domains are practically covered. A special notice for the international character of this issue, featuring contributions from Romania, India, Bulgaria and Turkey.

The article “Medico-legal aspects of dental practice”, by authors from the prestigious Sri Aurobindo School of Dental Medicine of Delhi, India, starts from the dental practitioners’ lack of medico-legal knowledge and tackles ethical issues in dental medicine, dilemmas regarding consensus, potential mistreatment, the diversity of medical decisions and complex implications in relation to the patients’ interests. The paper is worth reading by the dentists in view of the current widened range of therapeutic options, new technologies and increasing demands of the patients.

“Fecal transplantation: digestive and extradigestive clinical applications” belongs to the gastroenterology specialty. It was realized by a group of Romanian and Bulgarian research workers and it focuses on “fecal transplantation” as a procedure to restore bacterial population in patients with advanced enteral dysbiosis occurring in very severe conditions. We quote from the conclusions: “There is a tremendous attention in fecal transplant research for a multitude of digestive and extra digestive disorders associated with intestinal dysbiosis such as cardiovascular diseases, non-alcoholic liver steatosis, neuropsychiatric diseases, chronic acne, autoimmune diseases, renal and biliary lithiasis, metabolic syndrome with insulin resistance and many other diseases”. Even if fecal transplantation is currently used only in the treatment of entero-colic dysbiosis, in the next years it is expected to have extended applications and new practice guidelines.

A very interesting review deals with the role of Internet in the training and practice of nurses. It is entitled “Nurses and Internet health – related information: review on access and utility” and is elaborated by a group of researchers from Jordan. It is a well designed comprehensive article that demonstrates the utility of this fast and complex means of information and communication for the nurses. It is clearly shown how the nurses’ knowledge improves by the use of the Internet, whose practical role will continue to grow in the future.

The review “Current perspective regarding the application and incorporation of silver nanoparticles into dental biomaterials” is the result of a research conducted by authors of the Faculty of Dental Medicine of the Iuliu Hatieganu University of Medicine and Pharmacy of Cluj-Napoca, Romania. It is about a developing interesting field – of microparticles incorporated in dental material, being a comprehensive evaluation of the literature, well written, critical, with clear conclusions.

The last of the reviews in this issue refers to a pathology verging on two surgical specialties (maxillo-facial and ENT) – temporomandibular joint disorders. It approaches the diagnostic role of imaging methods. The authors, from the Department of Maxillofacial Surgery of the Iuliu Hatieganu University of Medicine and Pharmacy, analyze a large number of articles in recent literature and illustrate by various imaging procedures (conventional radiology, computed tomography, magnetic resonance imaging and ultrasonography) the complexity of the domain and the personalized approach necessary to treat the different disorders.

The original contributions begin with the study “Ferritin level changes and erythroid improvement in a group of adult polytransfused patients treated with Deferasidox” carried out on 40 patients polytransfused in the Department of Hematology of the Iuliu Hatieganu University of Medicine and Pharmacy. The authors find Deferasirox to be an efficient chelating agent of iron, as proven by the decrease of ferritin serum levels observed during treatment, as well as erythroid improvement in about one fifth of the patients.

The paper “Incidental findings during follow up in oncological patients”, elaborated by a group of researchers of the Department of Medical Imaging of the Iuliu Hatieganu University of Medicine and Pharmacy, evidences a high number of chance findings by CT scanning that are not related to the oncological disease. The conclusions emphasize the increased incidence of adrenal adenomas that raise difficult differential diagnostic problems.

Adherence to the information found on the Internet is studied in the article “Relationship of Internet addiction with depression and academic performance in Indian dental students” by a group of researchers in India. The importance of the study derives from the large numbers of users worldwide and the unrestricted access of adolescents to technology. They have a very high rate of adherence to the Internet, enhanced by mobile devices and this is associated with high rates of depression (1/5 students), which considerably influences academic performance.

Another aspect of communication, oncological this time, is the original study by a group of researchers from the Department of Public Health if the Iuliu Hatieganu University of Medicine and Pharmacy - “Improving communication within the interdisciplinary team monitoring young women with onco–gynecological pathology in Romania”. The starting point is the quality of communication between health care personnel and young patients with gynecological neoplasias. The analysis carried out in a large number of patients evidences the importance of multidisciplinary and online communication.

In the study “Lifestyle and psychosocial factors in musicians” the authors from the Iuliu Hatieganu University of Medicine and Pharmacy, Cluj-Napoca, Romania, emphasize the high levels of stress in beginner musicians and suggest solutions for such conditions.

The study ”Association between different blood groups, depression and oral health status of dental students” by authors of the Dental College and Hospital of Jaipur, India, shows that the DMFT score varies according to blood group, while depression, oral hygiene and gender do not influence this score.

The importance of traumatic dental anomalies is analyzed in a large group of patients by authors of Sri Aurobindo College of Dentistry. The social and economic impact of these disorders is emphasized in the study “Prevalence of traumatic dental injuries and their relation with predisposing factors among 8 – 15 years old school children of Indore City, India”.

The psychosocial aspects of orthodontic treatment are investigated in the article “Psycho–social impact of orthodontic treatment in Romanian teenagers and young adults” by authors of the Faculty of Dental Medicine of the Iuliu Hatieganu University of Medicine and Pharmacy. Over 50% of the patients treated reported a higher satisfaction regarding their facial aspect.

Finally in this issue you will find several interesting case presentations in the fields of dental medicine (“Cutaneous dental sinus of submental region: an eight years follow – up” – authors from Indore, India); cardiology (“Atrioventricular conduction defect associated with severe hyponatremia” – authors from Bulent Ecevit University, Turkey); laparoscopic surgery (“Retroperitoneal laparo - endoscopic single site approach for renal cyst decortication – first experience and review of the literature” – authors from the Department of Urology of the Iuliu Hatieganu University of Medicine and Pharmacy, Cluj-Napoca, Romania), all of them beautifully illustrated, well argued and of great interest for practitioners.

This issue finishes with a memorial of dr. Bârlea Gheorghe, a disciple of Gheorghe Bilascu, the founder of the School of Dentistry in Cluj-Napoca, Romania and a book presentation – “Anatomical Bases of Diaphragm Pathology” by Professor Francisc Grigorescu Sido, a prominent member of the medical academic community of Cluj.
